Affidavit of Residency Information

Please be advised that for the 2010-2011 school year, an Affidavit of Residence form is again required to be notarized and then submitted to your child’s school to show proof of residency.
This is a summary of the changes made in April, 2008, by the Fulton County Board of Education. No additional changes have been made.
- The only students who need to provide proof of residence with their affidavit are students who are new enrollees in a Fulton County school, students entering the sixth grade, and students entering the ninth grade. A parent or legal guardian of those students needs to provide two documents from the list provided on the affidavit form to show proof of residency. Along with the proof of residency documents, the parent or legal guardian must submit a completed and notarized Affidavit of Residence form titled for those circumstances.
- Students returning to the same school they attended last year and have not moved will not need to provide proof of residence but must complete and submit an Affidavit of Residence form (returning students) titled for that circumstance.
- Any student in his or her senior year of high school is not required to submit an Affidavit of Residence form.
- Each school will have a notary available to notarize the form at no charge to the family.
The notarized Affidavit of Residence form legally verifies that your child resides in the school attendance zone or has an approved hardship or other placement, for the school he/she attends.
